Job Description
The Family Nurse Practitioner will work with the Lead Provider to play a critical role in both care delivery as well as the ongoing design.
Specific tasks will include:
Delivering Primary Care within the practices, through both traditional face-to-face visits as well as non-visit based interactions (e.g. phone, email, groups, home visits, video chat). Collaborating with a team of health coaches, who, based on a community health worker will help patients to set self-management goals, improve patient self-efficacy, and support physicians.
Participating in development of robust group visit offerings, including educational classes, integrative yoga and fitness classes, etc.
Taking a population management approach to care by viewing the entire community of patients as recipients, and using technology and team resources to direct care towards those most in need.
Participating in communicating and marketing the practice model to new patients as well as other stakeholders such as the medical community, payers, and the press.
Carrying out other appropriate professional duties as may be necessary, including liaison with outside medical consultants, hospitalists, emergency room providers, etc.
