Provide health education and counseling to assigned patients by promoting positive health behaviors and self care skills. Provide family planning counseling. Provide care for Acute and Chronic Illness and Urgent Care services that includes assessment, diagnosis, treatment and monitoring of illness. Consult with physician partner and/or refer to subspecialty care patients with more complex/dynamic problems. Provide primary care and preventive services to children and adolescent populations within the assigned site/location as assigned by the designated clinical supervisor. Maintain and update Electronic Health Records (EHR) in a systematic format and complete required documentation within the specified amount of time expected by the assigned manager/director. Ensure that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment. Required Qualifications: Master’s Degree as a Family Nurse Practitioner. Active, Unrestricted Nurse Practitioner License in Massachusetts. Oncology Nurse Practitoner or Physician Assistant The Nurse Practitioner provides primary care in collaborative joint practice with the physicians to oncology patients. Scope of responsibility includes expert practitioner, educator/teacher, care management, consultant, nurse researcher, and nurse leader.  Assesses, plans, implements and evaluates care for individual patients. -Initiates and documents appropriate interventions; evaluates and records patient/family response within practice protocol; prescribes drug therapies per protocol/physician consultation. -Assists physicians and performs procedures as needed; conducts patient assessments and provides physician with information prior to seeing the patient. -Assesses patient/family needs and provides education. -Promotes wellness and recommends community resources to meet patient and family needs. -Promotes/provides staff education. -Incorporates evidence-based research into clinical practice. -Knowledgeable of reimbursement procedures and documentation requirements. -Completes patient charge tickets. -Provides case management for oncology clients and their families. -Serves as a resource person for the nursing staff by providing information being studied in clinical trials. -May supervise the activities of clinical nursing and support staff. -May mentor new mid-level providers MINIMUM QUALIFICATIONS: Masters of Science in Nursing (MSN) from an accredited program for nursing education and successful completion of a Nurse Practitioner program. Minimum of three years post MSN experience in oncology nursing. Experience and knowledge in the management of the physiologic and psychological effects of cancer diagnosis and subsequent treatment. RN state license and current registration with the State Board of Nursing in practicing state. Current state certification of Nurse Practitioner. Current CPR certification required.
