Job Description & Requirements APP: Nurse Practitioner/Physician Assistant - Primary Care in California StartDate: ASAP Available Shifts: 0.8 FTE Job Description This position is for an Advanced Practice Provider (APP) with Primary Care experience to provide coverage across 13 clinics within the department of Primary Care and Population Health with locations spanning from Palo Alto to San Jose.  The provider in this position will be scheduled in various clinics to provide coverage for other providers in the Primary Care Provider role who will be out of the clinic for an extended period of time (examples: Leave of absence, sabbatical, transitioning of provider). The APP will not build and manage his/her own panel of patients in this role, however, will help to support the panel management for the provider(s) he/she is covering - including same day and follow-up visits, chronic care management, physicals and preventative care visits, in-basket management, and other clinic support as indicated.  This position is best suited for someone who is a team player, yet enjoys working autonomously and likes variety.  Preferably, the APP will be able to see patients of all ages and in-clinic minor procedure experience is a plus.  The Nurse Practitioner/Physician Assistant (NP/PA) functions under the general Nurse Practitioner Standardized Process Protocol or general Physician Assistant Role Description approved by the Interdisciplinary Practice Committee (IDPC).
