The Speech/Language Pathologist obtains and documents appropriately all required and pertinent patient information; orients patients, family and other appropriate parties to the purpose and procedures of the speech therapy evaluation and treatment; selects and performs evaluations; develops an individualized patient treatment plan; documents and initial plan of care and completes certification; coordinates treatment schedules with disciplines; utilizes a team approach in the delivery and management of services and patient care; selects and provides treatment; administers tests, scores and interprets results; identifies significant changes in patients condition; provides appropriate patient/family education and teaching; documents the patients response to treatment and progress; completes all documentation; updates patient treatment plan and documents; meets all requirements of third party payors; completes patient charges, and meets productivity standards. Patients cared for include those in the following age group: children, adolescents, adult and geriatric. KNOWLEDGE & SKILLS REQUIRED: Required: Masters Degree in Speech/Language Pathology. Licensed by North Dakota in Speech/Language Pathology. Licensed by Minnesota in Speech/Language Pathology, if responsible for Outreach and Home Health. Certified by the American Speech Hearing Association (ASHA) with CCC-Sp or CFY with a qualified Speech/Language Pathology clinical supervisor and will obtain ASHA CCC-Sp within one (1) year from date of hire. Valid drivers license. Successful completion of the functional assessment. Part Time, Temp, or Per Diem Employment

Permanent / Full Time Rehabilitation Specialties Speech Pathologist (SLP) in Grand Forks, ND Permanent / Full Time Days - Refer to Job Summary for Qualifications The Speech/Language Pathologist (SLP) evaluates, develops, implements and documents an intervention plan; provides intervention, evaluating and documenting progress toward goal, modifying interventions methods, working with therapy team members to maintain patients goal as primary goal; develops and documents a discharge plan; provides supervision for therapy assistants and other aides; provides consultation and education for patient, families, referral sources and community agencies; participates in growth and development of service area, new programs, review of existing programs and maintains positive lines of communication with contract facilities continually promoting our team of therapy services. Patients cared for include infants, children, adolescents, adults and geriatric. KNOWLEDGE & SKILLS REQUIRED: Required: Masters Degree in Speech/Language Pathology. Licensed by North Dakota in Speech/Language Pathology. Licensed by Minnesota in Speech/Language Pathology, if responsible for Outreach and Home Health. Certified by the American Speech Hearing Association (ASHA) with CCC-Sp or CFY with a qualified Speech/Language Pathology clinical supervisor and will obtain ASHA CCC-Sp within 15 months from date of hire. Valid drivers license. Successful completion of the functional assessment. Preferred: Previous experience. SUPERVISION RECEIVED: Direct supervision is received from the Manager of Outreach Rehabilitation Services. Clinical direction is received from a CCC-Sp certified by American Speech Hearing Association (ASHA) with CCC-Sp.
