ESSENTIAL JOB DUTIES: Evaluates, plans and implements patient care programs through the use of selected Speech/Language Pathology procedures, MBS studies, and formal/informal tests. Directs, adapts and aids patients in therapeutic exercises and education for functional training/techniques. Consults with physicians in developing individual therapy plans designed to assist patients in reaching their maximum performance levels including appropriate use of skilled speech/language pathology services. Confers with and makes recommendations to physicians regarding the patients evaluation and total treatment needs to include appropriate use of Speech/Language Pathology services. Establishes individualized, written functional goals that are objective, measurable and realistically attainable in an appropriate time frame; incorporates patient/family goals. Instructs patients, families, nursing, and/or other personnel in speech/language/cognitive/dysphagia programs that can be done within and outside the Speech Therapy Department. Informs physicians, discharge planners and/or Speech/Language Pathologists at the next continuum of care of appropriate discharge planning needs for patients regarding related speech/language pathology needs. Evaluates, records and reports patients status for review by other Speech/Language Pathologists and members of the health care team. Maintains medical records and files reflecting up-to-date patient progress, reassessment, care plans and related documentation as required by hospital policies. Participates in case conferences and discharge planning activities. Participates in development and compliance with continuous performance improvement activities as well as Shared Governance. Assists with teaching activities that may include, but not limited to, orientation of new Speech/Language Pathology staff, students, CFY/RPE candidates, volunteers and medical staff. Assists with instruction to staff in the use of equipment, formal tests and/or procedures. Demonstrates professionalism in communication and compassion with regard to human dignity. Maintains high standards of conduct in hospitality, excellence, appearance, attitude, respect, and teamwork. Cooperates fully in all risk management activities and investigations (i.e., notification forms, etc.). Demonstrates responsibility in maintaining confidentiality of hospital information and data. Provide evaluations as ordered by physician. Provide treatment appropriate to evaluation findings and within the professional scope of practice and individual level of expertise. Update treatment plans as needed to meet desired outcomes. Supervise clinical assistants in their disciplines as per scope of practice/licensure guidelines. Communicate regularly with other team members and on call therapists. Ensure discharge order is obtained per facility protocol. Responsible for completing all required documentation in a timely manner (to include eval, reports, daily progress notes, discharge reports, monthly reports, daily service and labor logs, screening logs, forms and other documentation). Permanent / Full Time Rehabilitation Specialties Speech Pathologist (SLP) in Loma Linda, CA Permanent / Full Time Days - Masters Degree CPR/BLS 1-2 years Job Summary: functions as an integral member of a interdisciplinary team providing diagnosis and treatment recommendations in a coordinated manner with recommendations from the specialties. Has a working knowledge of the specialties of other team members involved in the care of children with craniofacial anomalies. Provides speech and language screenings as part of the Craniofacial Team Clinic within the time constraints of a busy interdisciplinary team clinic and identifies patients for whom further, in depth evaluations are necessary and provides appropriate referrals for additional assessment. Conducts age-specific and developmentally appropriate speech and language assessments, diagnosis specific communication impairments, develops treatments plans and goals. Delivers therapy and monitors progress using appropriate documentation. Provides early intervention programs. Acts as a liaison with community speech-language pathologists to coordinate treatment and provide specialty education. Utilizes discretion and independent judgment in performing all aspects of patient care. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills; utilizes knowledge in the delivery of quality whole person care; utilizes interpersonal skills to maximize customer service in a professional manner. Performs other duties as needed. Job Requirements: Minimum MA/MS degree in Speech Pathology or Communicative Disorders. Minimum of 1-year experience in a medical speech pathology or rehabilitation setting. Current Speech Pathology licensure and ASHA certification, CPR. Able to read, write legibly, speak in English with professional quality; use computer, printer and software programs necessary to the position; Demonstrates understanding of the economics of rehabilitation, (i.e. funding policies and sources, accreditation, marketability, cost management, managed care, case management systems, materials management, etc.) Cooperates with management and other associates in achieving organizational goals, clinical program, and mission.
