Provides counseling to patients and their families for prevention of illness and maintenance of health, in order to assist patients in attaining physical, mental, and social well-being. Oversees compliance with the preventive healthcare needs of adult patients visiting the Primary Care Clinic. Refers high-risk, unstable, or extremely complex patients to the physicians and emergency medicine. Performs emergency care as appropriate. Participates in patient care rounds and conferences in the outpatient setting. Presents patient cases and classes for patient care conferences and academic institutions. Communicates patient management strategies to members of the patient care team. Serves in a leadership position and is recognized as a consultant and role model to other health care professionals both within, and outside, the institution. Summary:

Health Center Supervisor/Clinician NP/PA manages, supervises and coordinates staff and health center operations.

Essential Functions:

  • Typically oversees the management of a single, small site (<10 headcount)

  • Identifies process improvement opportunities and presents resolutions and recommendations to the Director of Site Operations
  • Makes recommendations regarding staffing model based on objective scheduling & volume analysis presents to Director of Site Operations for approval
  • Understands and complies with all regulatory, procedural, policy and licensing requirements
  • Completes incident reports as needed and provides assistance to Medical Care and outcomes dept regarding investigations and the resolution of complaints
  • Communicates and coordinates corporate messages and ensures implementation of policies or procedures is followed
  • Coaches and provides feedback to staff on a regular basis
  • Manages internal budget, reviews expenses, purchases supplies, inventories equipment, medication etc.
  • Assists in the identification and use of local per diems to ensure appropriate staffing levels are met
  • Communicates regularly with staff, conducts meetings and shares information regularly
  • Works collaboratively with the Medical Leader at the site to manage internal site issues
  • Other duties as assigned

    Requirements:

  • Current license as a licensed Nurse Practitioner or licensed Physician Assistant in practicing state
  • Minimum of 3 years clinical experience
  • Certification in Occupational Health (COHN/COHN-S) may be required for some sites

    We are seeking a Full Time Nurse Practitioner to provide care between our 4 Health Centers located in the Houston, TX area!

    Responsibilities:

  • Provide general medical care and treatment to patients in health center
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Records physical findings, and formulates treatment plan and prognosis.
  • Provides written home-going instructions.
  • Discusses case with Physician and other health professionals to prepare comprehensive patient care plan.
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
  • Refers patients to Physician for consultation or to specialized health resources for treatment.
  • Educates and coaches nursing staff on best nursing practices.
  • Will alternate between four facilities in the Houston Area (LaPorte 25111, Channelview 25115, Bayport Choate 25118, Bayport Underwood 25117) All within @ 20 miles of each other.
  • Basic Life Support Certification
  • Current license to practice in the state
  • Experience in Occupational Health preferred
  • Good Computer skills
  • Master's degree (MSN) required.
  • At least three years of clinical experience as a Nurse Practitioner in an acute care setting.
  • Nationally Certified as a Family or Adult Nurse Practitioner
  • Appropriate certification to write prescriptions under the authority of the Site Medical Director, if allowed within state of practice.
  • Independent worker with exceptional organization skills.

  • Refers to external providers as needed
  • Facilitates early reporting of occupational illness or injuries
  • Collaborates with Infection and Communicable Disease Department in the management of infection control and medical surveillance of tuberculosis monitoring, needle stick injury tracking and monitoring, and outbreak exposure investigation, reporting
  • Provides workstation ergonomic evaluations providing recommendations for improvements, with follow-up evaluations after improvements are implemented
  • Participates and supports health and wellness initiatives, health education and screening programs
  • Provide utilization and participation clinic data
  • Masters degree, certification as a nurse practitioner with an adult population focus.
  • Three to five years clinical experience in acute care, ambulatory, or emergency care.
  • Excellent written and oral communication skills.
  • Experience in occupational health/corporate health/urgent care nursing preferred
  • Experience utilizing electronic medical records or databases
  • Current CPR or BLS certification required
