The purpose of the work is to ensure a high level of health care for primary care patients with chronic and acute illnesses. The incumbent is responsible for facilitating the medical care of the patients in the outpatient setting, in addition to developing the plan of care. This includes participation in patient education in order to promote the effective involvement of patients in their care. The work engages in Department of Army research with immediate and long-range impact through planning, implementation of care and evaluation, patient teaching, family counseling and involvement with interdisciplinary colleagues in the advancement of medical treatment initiatives.

Employee provides independent and collaborative specialty, comprehensive and therapeutic health care services, integrating the art and science of nursing into medical practice. Medical management functions performed include assessment of acute, non-acute and chronic clinical and psychological diagnosis, new patient intake/screening, history and physical examination, laboratory and diagnostic studies; to include interpretation, ordering appropriate medications and treatments, providing staff education, and the initiation of a therapeutic plan in accordance with medical practice. Formats a therapeutic plan of care in collaboration with specialty physicians. Implementation includes major emphasis in the areas of patient/family education, assessment, comprehension, and support. The incumbent is responsible for the initiation, implementation and supervision of treatment plans and education for chronic conditions such as; but not limited to, coronary artery disease, endocrine diseases such as diabetes, hyperthyroidism, obesity, hyperlipidemia, and chronic kidney disease. Collaborates with physicians and other team members regarding patient management, to include evaluating and revising therapeutic plans. Provides educational management to patients and their families regarding disease processes and diagnostic tests; the therapeutic regimen including medication, diet and follow-up care; methods of adapting medication and treatment to current lifestyles. Provides counseling to patients and their families for prevention of illness and maintenance of health, in order to assist patients in attaining physical, mental, and social well-being. Oversees compliance with the preventive healthcare needs of adult patients visiting the Primary Care Clinic. Refers high-risk, unstable, or extremely complex patients to the physicians and emergency medicine. Performs emergency care as appropriate. Participates in patient care rounds and conferences in the outpatient setting. Presents patient cases and classes for patient care conferences and academic institutions. Communicates patient management strategies to members of the patient care team. Serves in a leadership position and is recognized as a consultant and role model to other health care professionals both within, and outside, the institution. Acts as a preceptor to graduate students enrolled in nurse practitioner programs. Develops and/or conducts in-service education programs for other health care professionals. Serves as advisor/member of committees as needed. Develops, reviews, and appropriately updates policies, protocols, standards of nursing practice, standard operating procedures and scope of practice regarding adult, adolescent, pediatric and geriatric patient care in collaboration with the multidisciplinary members of the health care team. Performs other duties as assigned.

This person exercises judgment in identifying and resolving issues and problems. The manager applies both clinical and operational management skills to meet the established quality, patient satisfaction, and financial targets of the unit. This person accomplishes all of the above in collaboration and partnership with physicians.

Must possess a Missouri license as a registered nurse. Must have a Bachelors of Science in Nursing or actively pursuing a BSN. The position requires a minimum of three years experience as a staff nurse, within the past ten years. This person must have well-developed written and verbal skills. Will also have analytical and problem solving skills necessary to successfully manage the operations of a nursing unit on an assigned shift. We are currently seeking a Nurse Manager to be responsible for a 40 bed med/surg nursing division, and has 24-hour responsibility for patient care and overall unit operations. This person sets annual goals and operational and capital budgets. The manager ensures quality patient care is delivered consistent with established contemporary standards for patient care services.
