Fort Campbell/Clarksville: U.S. Army Medicine Civilian Corps

The purpose of the work is to ensure a high level of health care for primary care patients with chronic and acute illnesses. The incumbent is responsible for facilitating the medical care of the patients in the outpatient setting, in addition to developing the plan of care. This includes participation in patient education in order to promote the effective involvement of patients in their care. The work engages in Department of Army research with immediate and long-range impact through planning, implementation of care and evaluation, patient teaching, family counseling and involvement with interdisciplinary colleagues in the advancement of medical treatment initiatives.

Employee provides independent and collaborative specialty, comprehensive and therapeutic health care services, integrating the art and science of nursing into medical practice. Medical management functions performed include assessment of acute, non-acute and chronic clinical and psychological diagnosis, new patient intake/screening, history and physical examination, laboratory and diagnostic studies; to include interpretation, ordering appropriate medications and treatments, providing staff education, and the initiation of a therapeutic plan in accordance with medical practice. Formats a therapeutic plan of care in collaboration with specialty physicians. Implementation includes major emphasis in the areas of patient/family education, assessment, comprehension, and support. The incumbent is responsible for the initiation, implementation and supervision of treatment plans and education for chronic conditions such as; but not limited to, coronary artery disease, endocrine diseases such as diabetes, hyperthyroidism, obesity, hyperlipidemia, and chronic kidney disease. Collaborates with physicians and other team members regarding patient management, to include evaluating and revising therapeutic plans. Provides educational management to patients and their families regarding disease processes and diagnostic tests; the therapeutic regimen including medication, diet and follow-up care; methods of adapting medication and treatment to current lifestyles. Provides counseling to patients and their families for prevention of illness and maintenance of health, in order to assist patients in attaining physical, mental, and social well-being. Oversees compliance with the preventive healthcare needs of adult patients visiting the Primary Care Clinic. Refers high-risk, unstable, or extremely complex patients to the physicians and emergency medicine. Performs emergency care as appropriate. Participates in patient care rounds and conferences in the outpatient setting. Presents patient cases and classes for patient care conferences and academic institutions. Communicates patient management strategies to members of the patient care team. Serves in a leadership position and is recognized as a consultant and role model to other health care professionals both within, and outside, the institution. Acts as a preceptor to graduate students enrolled in nurse practitioner programs. Develops and/or conducts in-service education programs for other health care professionals. Serves as advisor/member of committees as needed. Develops, reviews, and appropriately updates policies, protocols, standards of nursing practice, standard operating procedures and scope of practice regarding adult, adolescent, pediatric and geriatric patient care in collaboration with the multidisciplinary members of the health care team. Performs other duties as assigned.

Provides comprehensive care for psychiatric patients based upon the multi-disciplinary treatment plan and the mental, emotional and physical needs of the patient and provides assessment of acute and non-acute clinical problems and toxicity's as they relate to a patient's psychiatric condition.
