He/She provides direct patient care as needed during his/her shift. He/She contributes to the vision, mission, and strategic plan of the organization and is accountable for the Standards of Care and practice in the area of responsibility where clinical nursing is practiced. The RN must demonstrate knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age, specific needs, and to provide the care needed as directed in the units departmental policies and procedures. ACCOUNTABILITY The staff RN is accountable to the Nursing Supervisor first and the Department Manager second. Permanent / Full Time RN Medical / Surgical in Lancaster, NH Permanent / Full Time Days - CPR Required POSITION SUMMARY: The Care Management Registered Nurse is responsible for overseeing all inpatients, observation patients and SNF patients from admission to discharge in a utilization review, discharge planning coordination and educational / support capacity. The nurse is responsible for utilizing the Nursing Process to ensure that quality care is provided to adult and pediatric patients as it relates to care management scope of practice, making quality referrals as appropriate. ACCOUNTABILITY The Care Management Registered Nurse is accountable to the Department Manager of the Med Surg unit. INTERRELATIONSHIPS Works closely with medical staff, hospitalist, Care Management colleagues, clinical staff, support staff, home health staff and physician office staff of Weeks Medical Center. QUALIFICATIONS 1. Current New Hampshire RN license. 2. Minimum 3 years medical-surgical nursing experience with knowledge of new trends and techniques in nursing. 3. Curent BLS certification required. ACLS preferred. 4. Case Management certification within 2 years of this position. Prior Care Management experience required. 5. Computer skills: Microsoft Word required; Microsoft Excel, Microsoft Access, email preferred 6. Knowledge and skills with Utilization Review criteria required. Knowledge and skill with CPT, ICD-9CM and HCPCS coding and it's relationship to billing preferred 7. BSN preferred ESSENTIAL FUNCTIONS Intake Coordinator: 1. Coordinate and assign patients admitted to a patient care area (including acute, ICU, observation and SNF levels of care) during working shift and assess all new admissions during off shift to ensure appropriateness of both status and placement. a. Emergency Room b. Physician's Offices c. E-Discharge or transfers from other facilities 2. Facilitate the IDT process and implement discharge plan and teaching for patients, families and nursing staff as necessary. a. Utilization Review/Discharge planning i. Monitor patients to determine if they meet our internal review criteria ii. Document findings of internal review in documentation tool utilizing appropriate software. iii. Work closely with hospitalist and IDT to provide the necessary clinical component to the discharge planning process iv. Communicate with the insurance company case managers as needed to provide concurrent review of inpatients as requested v. Assist with appeal process for denials from payors Outflow Coordinator: 3. Care coordination for Weeks Medical Center: a. Coordinate with patients, families and staff discharge goals and facilitate the transition towards discharge including: i. SNF Placement ii. Discharges to Assisted Living Facilities iii. Home Health coordination for discharges to home with services iv. Hospice Coordination b. Coordinate transfers from the facility including: i. Discharges ii. Hospital to hospital transfers 1. Emergent 2. Discharge related (see above) 3. Outpatient Appointments for inpatients 4.
