The Licensed Practical Nurse performs and documents assessments appropriate to licensure and ensures that the assessment is reviewed by a Registered Nurse; ensures a health history is completed; identifies discharge planning/education needs and plans to meet those needs; provides patient care; administers medication; performs pain assessment and takes appropriate action; identifies significant changes or high risk situations in patients condition; provides patient/patients family with appropriate education; ensures that intervention or referrals are made; evaluates and documents patient/patients family response to treatment, education and progress; completes all documentation; performs and/or assists with procedures; participates with multidisciplinary team; communicates patient progress; assists with coordination and preparation of transfer or transport; assists with the discharge process; demonstrates understanding and follows infection control policies and procedures; maintains requirements for age-specific competencies; and maintains all licensure, certification or registration requirements. Patients cared for include infants, children, adolescents and adults. KNOWLEDGE & SKILLS REQUIRED: Required: Associate Degree in Licensed Practical Nursing. Any LPN with acceptable certification may apply (includes any LPNs grandfathered into new certification requirements). Licensure required by the North Dakota Board of Nursing. Preferred: Experience in a health care facility. Part Time, Temp, or Per Diem Employment
